MUST BE A FULL FARE TICKET

NO BUDDY PASSES OR MILEAGE TICKETS

3.    1^st Departing Date

4.    Confirmation Code

5.    Phone Number so we can contact you if needed

6.    If you send in Mileage or Buddy Pass tickets you will be placed on a permanent ban list from Hawaiian Airlines Free Board Bag Waiver through NSSA HAWAII

(2 Surfboards ONLY!!!)

7.    All LATE submissions (after 7:30am) 1 week before contest will not submitted
